2024 System Updates

14/11/24
  • We have changed the Show Assignments icon and text in the Manage Users window, to make it clearer where to click when editing user assignments.
  • We have clarified that the top 10 is a read-only list with the ability to add comments. If edits are needed, Risks can be edited in the Activities window.
  • Added the option to prevent non-admin users from amending the due date on general actions.  
  • We have added an option to show only summary actions in the Activities window.
  • We have fixed an issue with date filtering in the Activities window.

  • Fixed an issue when multiple impacts were selected it was pulling through the options from the threat matrix rather than the opportunity matrix.
  • Fixed: Non-admin users were not able to edit IQs that were in draft

  • We have fixed the issue whereby, the Approve or Reject option incorrectly disappears after pressing cancel on a general action.
  • We have added a new query module graph that shows the current assigned user.

  • A show references button has been added to the risks matrix.
  • Fixed the DCWS lessons import and improved the export speed.

  • The login page has been streamlined and links have been added to the company website, system updates page and Linked-In.
  • We have made a change to allow approvers to progress workflows in IR and TQ modules.

  • TQs and IQs can now export PDFs.
  • Some bug fixes for the rich text editor, especially around bullet points and returns.

19/09/24

  • Reduced the loading on activities pages for when companies don’t actually use risks; just general actions or vice versa.
  • Additional date filters have been added for activities, risks and general actions.

12/09/24

  • An option has been added in the Admin Manage User dialog to show all items that a user is assigned to along with the option to export to excel.
  • PDF exports available for MOCs, IRs and Advanced TQs.
  • User dashboards are created with all sections enabled by default rather than requiring the user to choose.
  • Consistency changes to align IRs and Advanced TQs

  • Decision and Change Management items and their actions can be added to the User Dashboard.
  • In Lessons Learned, a bespoke change has been added to allow lesson approvers to bypass the draft stage in making a new lesson.
  • Bug fix for TQ’s sometimes not showing on dashboards.
  • Bug fix for closing comments not appearing in a closed lesson action.
  • Individual risks now have a score history chart.
     

22/08/24

  • Fixed a bug that now allows non-Prism admins to add user delegates.
  • From the Lesson Actions page and opening a parent Lesson, the next and previous buttons are now useable there.
  • IR areas can now be linked to activities.
  • When going into a new Lesson from the projects page, the project will be pre-selected for you.

15/08/24

  • Additional filters have been added for the audit log to filter by module and certain action types.
  • We’ll add an extra option to show risks for ALL activities.
  • Risk Actions now have a history table that tracks the due dates, when they were changed and why.
  • If you navigate away from a risk using the next and previous buttons while you have unsaved changes, you will be prompted with the classic “You have unsaved changes. Are you sure?” message.
  • Some bug fixes around the Top 10 Risks, their sorting and filtering.
  • Some minor bug fixes around the risk score ‘justification’ changes.
  • For all modules now, you can now ‘attach’ documents to them before saving an item for the first time.
  • Tidied up the risk review date process. This also helped remove an issue where saving the review wiped any unsaved changes elsewhere.
  • If comment threads get really long, their screen space is kept to a fixed size but they can be scrolled along.
  • The ordering of comments can be swapped around to show the most recent additions first.
  • Extra bug fix on Lessons’s “Add Another” button.

08/08/24

  • If you jump between risks with the next and previous buttons, if you have made any changes, you will be given a “You have Unsaved Changes…” message.
  • The same “You have Unsaved Changes…” has been applied to general actions.
  • Fixed a missing images reference in the system guide.

01/08/24

  • Performance improvements to the ‘All Risks’ exports. 
  • You can now upload documents to risk actions, general actions, lessons and lesson actions, IQs and TQs before saving them for the first time.
  • The system guide has been updated to include information on the risk history chart and the user delegations.
  • Some bug fixes around permissions not determining correctly when using the activity module’s next and previous risk buttons.

25/07/24

  • In Lesson Actions, the export button has also been moved to a location that makes it easier to see.
  • Dynamic charts have been added to existing modules.
  • For MOCs, you can now copy-paste formatted text (such as bullet points) into the text boxes. 
  • Fix on error messages for activity references.

11/07/24

  • Fixed the close button for risks not working on the user dashboard.
  • MOCs now have a PDF export.
  • Individual risks can now show a complete score history of that risk. Click the button above the risk matrix while inside a risk to see it.
  • Users who change a risk score will now have to provide a reason why they did it. This will also show on the risk history.
  • Risk and action categories that aren’t actually needed for their respective sub-modules are removed from import templates.
  • Lessons Learned now have next and previous buttons.
  • IQ Actions have been removed from the user dashboard to avoid confusion on their purpose.
  • User delegations now work for Lessons Learned.

04/07/24

  • The recently added user delegations feature can now be used for risks, risk actions and general actions.
  • Improvements to linking decision records and MOCs.
    The MOC dashboard now has new KPI charts; one that shows how long MOCs are from being created to closed. The other shows the proportions of MOCs that are successfully closed before their target date versus those that aren’t.
  • Some bug fixes for risk matrices and less-commonly-used scoring systems in Top 10 Risks.

27/06/24

  • MOCs now have greater flexibility in their editing. Certain users can now modify the contents of MOCs, and change their reviewer lists, right up to and including the Pending Review and Approval stages.
  • On the user dashboard, MOC actions are more visibly separated between open, overdue and draft in status.
  • A new ‘landing’ page has been added for the Lessons Learned module. This change is on a per company basis and can be turned on in the admin page by administrators. This feature greets users with a list of lesson projects. You can then click into the project to see your usual lesson list.

20/06/24

  • In the Admin module, under Manage Users, you can now elect ‘delegates’ for other users. A ‘delegate’ user will have all the same editing, approving etc. privileges of its ‘parent’ user if they are assigned a role in something. 
    • Rollout for other modules to come!
  • Daily reminder emails for users can now be collated into single summary emails, allowing a user to only get one email from Prism Apps each morning rather than potentially dozens. This setting is adjustable on a per-company basis.
    • All existing companies will, by default, still get the individual reminder emails unless someone switches the setting on. Get in touch through info@prismenergy.co.uk if you would like this enabled.

13/06/24

  • The MOC area pages have a couple of new columns. The created and (where applicable) target closure dates are now visible here. Same goes for the exports as well.
  • MOC export performances have been greatly improved upon.
  • In Lessons Learned, if the user only has one option under ‘More Options’, usually the export button, it will show by default instead of ‘More’.

  • The MOC exports can now also include attached documents from the underlying actions.
  • MOC exports can also include the statuses of their underlying actions.
  • MOC action comments that aren’t saved will now be saved when saving the parent action.

30/05/24

  • Fixed a bug that occasionally stopped the “Add” process on user management from working in the admin page.
  • If using additional approvers in general actions, the ‘final’ closure approver will not be notified by email until all other approvers have given their approval.
  • Styling tidy-up of the multi-approvers parts.
  • MOC exports can now include comments from the underlying actions in their exports.

23/05/24

  • Reminder emails for MOCs will no longer go out to reviewers if the MOC isn’t closed but they have nothing more to do.
  • MOCs in the ‘queried’ status can now be edited!

16/05/24

  • The ‘All’ pages for IQs, TQs, IRs and Advanced TQs all have filters now.
  • Exports in the ‘All’ filters will now use the filter selections beyond just the status.
  • If an IR’s respond-by date is changed, some of the roles within that IR get an email telling them about the change and who did it.

09/05/24

  • Clickable charts that take the user to the all actions page now automatically apply the relevant action type.
  • MOC areas can now be closed by admins, moderators and the responsible user.
  • Reduced number of email notifications received from the MOC module
    • Owner changed emails no longer sent.
    • If an MOC has been rejected the approaching closure date email is not sent.

  • The Lessons Learned page has had a tidy-up of the elements on the main page, reduced clutter and white-spacing.
  • General actions lists and All General Actions now have a search bar.
  • The MOC Areas page now has ‘clickable charts’. Clicking on a bar on the bar chart will take you to another page.
  • The All MOCs page can now be filtered by owner user.

23/04/24

  • Timewriting can show the last four months of backlog rather than two months, for logging time.
  • IR and Advanced TQ notifications have been trimmed down to reduce the risk of regular users getting overwhelmed with emails.
  • If closure approval is not enabled within an MOC area, the approver option is hidden.
  • Fixed a bug that caused new users to be accidentally added to the system via role assignments.
  • The system guide now contains details on the multiple risk matrices feature.
  • If closure approval is enabled for an activity, non-recurring general actions can have additional closure approvers.

11/04/24

  • Timewriting can now keep track of already-allocated hours and warn you if you’re going over.
  • Closed activities are now hidden from activity filters.
  • A fix for multi-impact risks not showing correctly on the All Risks page.
  • A fix to allow companies without Risks enabled to still access the All Actions page.
  • Small updates to the system guide, including adding a part about activity categories.
  • Bug fix for risks and actions; if no user is chosen, then the non-existent user will not be flagged as not being a team member.

  • In risks, risk actions and general actions, users will be warned if someone is getting assigned to a role when they’re not part of the activity’s team.
  • Some styling fixes for activity pages where companies do not always have risks and general actions enabled.

28/03/24

  • Activities can now have custom categories, just like risks, general actions, MOCs etc. 
  • The User Dashboard has been updated so that the edit, close, re-open etc, permissions are the same as if you were navigating to them normally. This currently only applies to owned risks, risks pending closure, owned risk actions and risk actions pending closure. 
  • SCRs now require a final ‘stamp’ of a user for submission and closure.
  • Fixed a bug where underlying code was appearing in tool-tips for the charts.
  • Fixed a bug where companies using the 1-9 scoring would lose their special scoring effect when using filters.

21/03/24

  • Time-Writing is now useable!
  • MOC Areas, IQs and TQs now have different graph colours to reflect their urgency.
  • A bug fix to limit the number of users that show on risk actions graphs.
  • A small bug fix to ensure that all users are selectable for MOCs and NCRs.

14/03/24

  • Lessons that are closed without being approved will correctly show as ‘closed’ in filters and exports.
  • Fixed a bug in being unable to open risks from the All Actions page.
  • When navigating the home dashboard and activities page, restricted users can now only see data and users pertaining to their assigned activities.
    • This covers graphs, filters and exports.
  • Comments have been added back to SCR actions.
  • Fixed a bug that prevented risks from being copied to other activities.
  • IRs, TQs and IQs can now be deleted.
  • When viewing a risk, the matrix inside the risk pop-out will only show the current risk.
  • When adding a new user, you can also add them directly to activity teams.
  • Ensure that, from the company select screen, companies that a user is disabled from should never be selectable.

07/03/24

  • The long-awaited Multi-Matrix feature is now live! Companies can now create threat and opportunity matrices that are in different scales to the original or ‘parent’ matrix. All companies will, by default only have one threat and one opp. matrix, but admins can add more if needed.
  • Companies using the one-to-nine risk scoring should now no longer have their risk lists randomly rearrange when they change a risk or create one.
  • In the MOC module, any filter selections will now affect the exports. The exports should reflect whatever is visible after filter selections.
  • In NCRs, any settings that hide the classifications pie chart will still show the classifications dropdown.
  • The ‘Current Assigned User’ is now a column with IR exports.
  • IQs and TQs now show their closed date when applicable, including in exports.
  • In the SCRs, ‘Maintenance Manager’ is now called ‘Onshore Execution Lead’.
  • Some small bug fixes for the home dashboard and their exports.

29/02/24

  • MOC Categories will now have a customisable ‘Visible in Filters’ option. Too many category filters showing for your company? Not a problem! Now you can toggle each one on and off for your searches. All of them will start with ‘true’ by default.
  • The PDF export for the home dashboard now shows the Top 10 risks’ references and titles / descriptions.
  • When a Wells Lesson is copied to regular Lessons Learned, a ‘copied from’ field is now visible, showing and linking to the original lesson.
  • Lessons Learned and Wells Lessons now use their created date for their listings, and said date is now visible in their exports and individual pages.
    • The original ‘date’ field these lists used has been clarified as the ‘last modified’ field.
  • The statuses of Lessons and Wells Lessons now appears in their exports.
  • Wells Lessons will have their references start with “WLL” rather than “LL”.
  • Fixes for email links generated throughout IQs, TQs, IRs and Advanced TQs.
  • Regular admin users can no longer create service accounts. Get in touch with the team to find out more.
  • The users of the SCRs have had the quantity of email notifications they receive and send scaled back.

22/02/24

  • Category Filters for MOCs have been added to the MOC dashboard and all MOCs pages.
  • MOCs that have been linked to an Activity can now be unlinked.
  • The IQ, TQ IR and Advanced TQ Exports now have a status column included.
  • The Top 10 Risks on the Risk Dashboard no longer include those from closed activities.
  • Add an option to MOC areas to make each area only available to its listed team members.
  • When using SSO the system now remembers your requested link if you are forced to re-authenticate.
  • Fix to ensure themes and categories are repopulated when switching between activities.
  • Fix to prevent the resend verification email from appearing on the MOC Team Member list.

15/02/24

  • MOC exports now create an overlay that stops the same user from clicking multiple times for one export.
  • MOC exports have their category selections added.
  • Fixed an issue that caused an error to appear when changing a risk’s priority.
  • Fixed the display of compact risk registers when the priority arrows are visible.
  • Fixed a bug where the user dashboard won’t load correctly if risks aren’t enabled.
  • On the user dashboard, SCR Actions have now been added as a list option.

08/02/24

  • The ‘All General Actions’ page now has the same more granular date filters that risks have. You can filter by dates created, last modified, etc.
  • The risk dashboard now has a PDF export.
  • User accounts now have a field called ‘Sponsor’ that admins can add a person’s name to. 
  • Removal of the remaining references to ‘Change Requests’. This is what Management of Change or MOCs used to be called.
  • Flight times can now be added to IAP forms. 
  • Fixed a bug that prevented companies that didn’t use risks from accessing the Admin Dashboard.
  • Some buttons in the SCR module have been renamed to be more easily understood.
  • Fixed a bug for MOCs that prevented users from saving an MOC as a ‘draft review’ when the owner, raised by user and approver were all the same person.
  • Fixed a bug that prevented email links from correctly opening for users that were using single-sign-on (SSO).

01/02/24

  • Risk scores now update their colours in the risk registers if the risk score changes.
  • The ‘Save and Close’ button in risks has been reworded to ‘Close Risk’ to help prevent any confusion in its purpose.
  • Fixed an issue in general actions where the actions list would randomly rearrange itself when opening or closing an action.
  • Fixed the same issue for risks as above!

25/01/24

  • Users can now close risks within the risk popups themselves rather than having to navigate out of them.
  • Fixed an issue where risk lists don’t always update when you change a risk while in the activity page.
  • The ‘Filter by Favourites’ option on the home dashboard is now available within the Activities page!
  • IRs now have a system guide page. The TQs one has been touched up too.

18/01/24

  • MOC’s can now be saved as ‘draft reviews’. 
  • Our dates “to and from” filters are now inclusive, so for example, searching for items between the 15th and the 18th will show relevant items for the 18th as well.
  • Minor bug fixes for IRs.
  • If you try and delete a company area that is being used for an IR Area, you will receive a notification
  • In NCRs, the pie charts for classifications will be hidden for users using impact-driven NCRs

11/01/24

  • “Actions” within IR’s have now been renamed to “Workflow Tasks”.
  • For submitting draft responses to the approver within IR’s, you can now create a draft response AND send it to the approver with just one button click.
  • Admins can now change role allocations within IRs.
  • IR Actions can now be viewed from the ‘All Actions’ page 
  • IR Areas can now be allocated to company areas similar to normal IQs and activities.
  • Added measures to ensure that if an IR is finally closed, that all its underlying actions are closed too.

2023 System Updates

07/12/23

  • On dashboards that show risks, and within activities, there are extended options now for filtering by certain dates. These include the created date, the last time the status changed, when it was last edited and when an action within was last edited.
  • Fixed a bug with the date selection for filters on the user dashboard.

23/11/23

  • Email links for general actions will now let you navigate directly to them! It will open the parent activity and then show you the action (older email links will still work but only new ones will open the actions).
  • Fixed a bug that prevented new general actions from being opened.

16/11/23

  • Risks, Risk Actions and General Actions can now only be deleted by Admins and Moderators.
  • Fixed an issue where risk review emails were still being sent after the risk had been closed.
  • The admin user export has a new column showing when users were disabled within their company.
  • There’s a new section in the Lessons Learned system guide explaining the different roles in the module.
  • Fixed a bug where an error would occur when trying to add a new impact type to the risk matrix.
  • The activities menu item is no longer shown if the company isn’t using the Risks or General Actions modules.

09/11/23

  • Recurring general actions are now available to those who want it! Routinely generated actions with different due dates can be created, and with a few choices of notification settings to go with.
  • NCRs can now be printed out in PDF format.
  • Fixed a bug for SCRs (formally called ‘Shutdowns’) where duplicate areas were being created.
  • On the main dashboard, you can click into / drill into the charts! This will take you to the relevant activities / actions pages with filters applied.
  • tidy-up of the ORA / QRA module (formally just ‘OQRA’) to tidy its appearance and remove parts no longer needed.
  • Lessons Learned ‘layered’ categories can no longer save duplicates of themselves.
  • Wells Lessons now have their many, layered categories available to be displayed in exports.

27/10/23    

  • A link has been added to the navbar menu that takes the user to the system updates page on the Prism Energy Website.
  • The charts on the risk and general actions dashboards have been made clickable. Clicking a segment on the chart will take the user to the all-risks or all-actions page with filters pre-applied based on what the chart displayed.
  • The activities page charts have also been made clickable and undergone a styling change.
  • OQRA Revisions can now have their own set of team members.

19/10/23

  • OQRA risks now have a ‘notes’ field, and their comments are now visible in their risk registers.
  • OQRAs (As in ORAs and QRAs) can now be deleted.
  • Risk Mitigation Descriptions can now be 2000 characters long instead of 1000.
  • The SCR link in the navigation bar will now only show for admins.
  • Notifications for the IAP module.
  • Bug fixes.
